Aggregate pricing means Refill negotiates medication cost across clinic volume pooled through its network, then passes those pre-negotiated rates to your account. The software fee layer scales with your plan: published tiers show transaction fees roughly between 1.5% and 5%, on top of whatever monthly platform fee your tier carries. If you add Refill Connect for a white-label patient portal, assessments, and billing, that is a separate monthly line starting around $399 plus setup and transaction fees. Optional provider-network encounters are priced publicly at about $30 for hands-on and $50 for hands-off per encounter when you outsource prescribing.
Because medication rates are not on a fully public catalog, your evaluation has to be question-driven. Ask for written per-vial cost on your top three SKUs at demo, confirm which plan tier sets your software fee percentage, and ask whether the meet-or-beat guarantee applies to your formulary and volume. Ask what is billed separately so the quoted medication rate plus software fee plus shipping reflects true landed cost, not a headline aggregate number.
